Online Preliminary Application
LINC has created a portal for parents to start the process for enrolling their children in LINC programs. This application is not complete without an in-person parent/guardian interview.
This online form begins the application process.
Completing the online application is the best way to reserve a spot in the program, but it does not guarantee admission to the program.
An in-person interview is REQUIRED to complete full application into the Caring Communities After School Program.
After your preliminary application has been submitted, your LINC Caring Communities site coordinator will contact you to arrange the interview.
Sites will hold in-person enrollment events in August prior to the beginning of school. Events will be posted below as they become available. Enrollment takes place at the school unless otherwise noted. Enrollment times are also available by appointment with your site coordinator.
Enrollment is required for participation in the program.
What you will need for the application:
Child Enrolling: Basic information about the child
Parent/Guardian: Basic information including employer, work hours
Emergency Contact: Contact information for at least two individuals
Medical Information: Doctor/clinic including phone and hospital
Required information is marked on the form.
What you will need to finish enrolling:
Immunization records
If your child has a diagnosed medical condition, an ICP (Individualized Care Plan) from your child’s doctor.
5 to 10 minutes for parent interview
