After you submit the online form:
Your school’s LINC Site Coordinator will contact you to schedule your interview.
In-person enrollment events will be held at schools in August, or you can set up an appointment.
Child Info: Basic details
Parent/Guardian Info: Contact, employer, and work hours
Emergency Contacts: At least two names and numbers
Medical Info: Doctor, hospital, and phone number
Completed online application
Immunization Records
ICP (Individualized Care Plan) from a doctor, if your child has a medical condition.
Brief in-person parent interview
