LINC Online Pre-Enrollment

Information needed to complete includes:

  • Child: Basic information about the child

  • Parent/Guardian: Basic information including employer, work hours

  • Emergency Contact: Contact information for at least two individuals

  • Medical Information: Doctor/clinic including phone and hospital

  • Other information is also requested. Required information is marked on the form.

LINC has created an online pre-enrollment portal to help parents enroll their children in LINC afterschool programs.

There is no enrollment fee or weekly/monthly charge for the program.

Enrollment is required to participate in the program.

The online pre-enrollment form requests basic required information.

Completing the form is only a first step to program enrollment.  

Times and places to complete in-person enrollment are available at the school or listed here.

If you are unable to complete the form online, you can complete the full enrollment process during the required parent/guardian interview.

IMPORTANT!

  • The form will take 10-12 minutes to complete.

  • The form must be completed in one session. There is no option to save and return later.

  • A separate form must be completed for each child.

  • A face-to-face interview is also required to complete enrollment.
